JOB REQUIREMENTS: Position Description: The Medical College of
Wisconsin\'s Department of Neurology is seeking a Nurse Practitioner or
Physician Assistant to provide primary or specialty medical care within
defined scope of practice under the supervision of a licensed physician
and in accordance with MCW/FMLH/CHW policies and procedures. This
position is located in Mequon, WI. The Advanced Practice Provider (APP)
works as a member of a multidisciplinary health care team who will
contribute to excellence in program administration, patient care,
patient safety, research, quality improvement, and teaching. The APPs
will provide professional healthcare to patients as directed by a
licensed physician and in accordance with policies and procedures.
Collaborate with Headache Medicine providers and clinical teams to
develop independent practice within the programs. In this role you will
be responsible for the following: Evaluate patients and render care,
within the scope of practice outlined by state statute, in the
ambulatory setting. Perform examination of patients and obtain/document
information regarding patient history. Order, perform and interpret
screening and diagnostic procedures and tests and treatments, and
appropriately communicate and document information. Evaluate and modify
healthcare plan as related to patient compliance and outcomes, under the
supervision of a physician. Participate in quality assurance activities
and/or clinical trials. Maintain detailed and confidential medical
records in accordance with established protocol. Acquire information on
the most current developments and trends in the Nurse
Practitioner/Physician Assistant field, through educational programs,
in-services, professional associations and literature. Write medication
and other orders as required for the management of patient care, as
directed by a physician, and when necessary. Administer botox
injections.Core Advanced Practice Responsibilities: Perform
comprehensive histories and physical exams, identifies normal and
abnormal findings, including physical, cognitive, and psychosocial
status. Order, perform, and interpret screening and diagnostic
procedures and tests, and appropriately communicates and documents
information Write accurate and timely progress notes, procedure notes,
and/or admission and transfer orders Develop pharmaceutical and
non-pharmaceutical treatment plans. Monitor effectiveness of plan of
care Educate patients on disease and treatment plans. Perform procedures
as trained and appropriately credentialed. Document within the
