Description
We are offering a contract to permanent employment opportunity for a Case Management Assistant - Administrative in San Leandro, California. This role supports the Care Management Team in administrative tasks related to care coordination and discharge planning activities. You will be working within the healthcare industry, providing essential support to ensure efficient and effective care management.
Responsibilities:
Coordinate with referral sources regarding bed availability, new products, and services
Provide administrative support for care coordination and discharge planning activities
Collaborate with Case Management staff to provide specific clinical information for initial and concurrent utilization review
Maintain a current database of existing and potential referral sources
Function as a key point of contact between Care Management staff, admissions, and payers
Coordinate and track any communication such as Important Message letters, Denial Letters, patient choice forms, and regularly update the Care Management team
Manage multiple inquiries and provide appropriate responses in a timely manner
Ensure accurate and critical information data entry and tracking, and report creation
Coordinate and obtain authorizations for admissions, document all information in the financial system, and work closely with Revenue Cycle to ensure each inpatient encounter is accurate
Communicate status with Care Management staff and arrange for patient transfer functions.

Requirements - Demonstrated proficiency in Customer Service
Strong Communication skills, both written and verbal
Experience with Data Entry tasks
Ability to handle Documentation processes efficiently
Proficient in Microsoft Excel
Knowledge of Procedures and Policy within a healthcare setting
Ability to Schedule Appointments effectively
Proficient in Microsoft Word
Experience with Database management
Proficient in Microsoft Outlook
Experience with Auditing processes
Understanding of Medical Coverage policies
Familiarity with Reconciliations process
Ability to handle complex Paperwork tasks
Ability to Perform Ad Hoc Financial tasks
Proficiency in English Language
Familiarity with the operations of a healthcare Facility
Proficient in Microsoft PowerPoint
Understanding of healthcare Practices and standards
Excellent Communication Skills
Knowledge of Clinical Trial Operations
Proficiency in Microsoft Office Suites
Experience with handling Medical Records
