Transporter furnishes transportation for Yellowhawk Tribal Health Center
patients/clients to medical appointments and treatment facilities. All
duties and responsibilities will be performed in accordance with the
Yellowhawk Mission and Vision statements. Essential Job Functions: \'
Transport Yellowhawk patients/clients to and from appointments and
treatment as assigned by the Transportation Coordinator. Ensure
compliance with the Yellowhawk Transportation Policy. \' Assist
patients/clients in and out of wheelchairs, cars, etc. \' Transporter
will not make exceptions nor deviate from the scheduled transports
without specific permission from the Community Health Nursing
Supervisor. \' Transport patients/clients to and from dialysis treatment
center as assigned. \' Transporter may transport patients/clients 400
miles or more daily as assigned. \' Document each transport on the
approved transportation form. \' Clean all Community Wellness GSA
vehicles and ensure maintenance and upkeep. \' Maintain confidentiality
in accordance with the Yellowhawk Tribal Health Center Policy Manual,
HIPAA and the Privacy Act. \' Other duties as assigned. Minimum
Qualifications \' High school diploma or equivalent. \' Must possess or
receive CPR and First Aid certification within ninety (90) days of
employment. \' Must possess a valid driver\'s license and maintain
insurance requirements to operate General Services Administration
vehicles. \' Must submit to and pass a reference and criminal background
check. \' Must submit to and pass a pre-employment drug and alcohol
screening. Preferred Qualifications \' Patient transporting experience
preferred.
