Job Summary
We are seeking a Nurse Practitioner (NP) to join our growing mobile wound care team. This is a flexible, field-based role where you’ll visit patients in their homes, assisted living facilities, or residential care settings to provide advanced wound care services.

Job Duties
- Perform wound assessments and develop treatment plans
- Provide direct wound care (including debridement, dressing changes, and skin substitute applications)
- Document visits and procedures in our EMR System
- Educate patients and caregivers on wound care techniques and prevention
- Collaborate with physicians, care teams, and facilities to ensure coordinated care
What We’re Looking For:
- Active Nurse Practitioner license in NJ
- Experience in wound care, home health, or geriatrics preferred
- Wound care certification (WCC, CWS, CWCN) is a plus, but not required
- Self-motivated with strong time management skills
- Comfortable working independently in the field
- Reliable transportation and valid driver’s license
