A large managed care healthcare organization with a strong reputation in NY is currently seeking Nurse Care Managers to join their team. Reporting to the Supervisor of Care Management this position will provide case management to the organization's members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the care plan, in conjunction with the PCP, caregivers and other team members, to help meet the member's needs.
Responsibilities:
- Provide case management to members, including medical, social, psychological, and physical
- Develop, implement, and monitor the care plan to help meet the member's needs
- Perform other duties, as needed
Qualifications:
- Managed care case management experience
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
Job Type: Full-time
Pay: $90,000.00 - $110,000.00 per year
